Support coordination is a capacity building support. This service is part of some participants plans to help achieve the goals that are set in their NDIS plan.
There are three levels of support coordination that can be included in participants plans
Support connection – Is the connecting of participants to supports outlined in participants’ plan.
Support coordination – Is assisting participants to understand and to build the skills to use their plan, ensure participants’ supports are being used to the fullest, track how participants are going with their plan and determine new goals with participant if required, promote independent decision-making, coordinate, and liaise with service providers.
Specialist support coordination – Is the highest level of support coordination. These participants have exceptionally high needs or are in a crisis situation, requiring for specialist support coordinators to have qualifications and experience to manage these needs.
Good Support coordinators should support participants to make their own choices and have control over their plan and never make decisions on their behalf.
They will find solutions to any issues they may be having, they will provide continuous follow ups with participants and connect participants with people in your family, friends, carers and keep everyone in the loop
Participants can change support coordinators at any time and over time may be confident enough to manage their supports independently and will no longer need support coordination in their plan

- Service provider directories
- Or Web search
