STEPS WE TAKE WITH YOU

Step 1  What are your goals in your NDIS Plan?

We will discuss your goals and what’s important to you.

STEP 2 Are there supports needs in your plan where Give a Care can help?

Your NDIS Plan has a lot of flexibility and this can be explored.

STEP 3 What daily living, healthy lifestyle, community activities or special interests are relevant and interesting to you?

We will have a discussion on the range of daily living options, recreation activities, community connections and upcoming events that may interest you.

Then you make the decision, it’s totally up to you.

STEP 4 We agree on the type of support, days and hours which fits you and your plan.

STEP 5 Your new support worker will support you as required, on the days and hours agreed upon.

It’s important your support worker is a good match for you. We have several support workers with a range of expertise and skills. We are confident there’s a good match for you!

STEP 6 We will review your supports with you every 6 months, or more regularly as your needs change.

 STEP 7 We will provide feedback of your goals and outcomes.

Throughout the process you should be thinking about your personal goals – remember you can contact us if you aren’t sure about something or need more detailed information for your NDIS forms.

Step By Step Guide To Your NDIS Plan

National Disability Insurance Scheme (NDIS) is a big revolution to how people with disabilities access supports and unlock a new world of prospects. Whether you want to become a participant of NDIS plan or would like to support your loved one, our step by step guide will help you getting started and get the most out of this scheme. 

If you would like to receive our assistance to access your NDIS or need information about NDIS forms, feel free to contact us. We will allocate time to streamline the process and ensure that your current & future needs and follow-up care is covered under your NDIS plan. 

Become an NDIS participant

  • Check Your Eligibility

Call the hotline on 1800 800 110 or explore www.ndis.gov.au to verify if you are eligible for the NDIS program. After completing the eligibility check, request the forms.

  • Submit The Forms To Become An NDIS Participant

If you are already a participant of NDIS scheme and need any specific support, you can approach your current provider who can help you filling the necessary Access Request Form.

– Once you have got NDIS approval, please let us know and provide us with a copy of it. 

– We will add up your requirements in compliance with your approved plan.

Steps to access the NDIS

If you are receiving funded disability supports now

Your information will be entrusted to the NDIA

The NDIA will either send a letter to you or call you to make an access request.

If the NDIA requires any other details, you will be sent a Supporting Evidence Form along with sections to be filled in by an educator, health professional or your GP. 

You must agree to become an NDIS participant by saying ‘Yes’. Once your plan is approved, state funding will be stopped.

If you are not receiving any disability supports

You can call the NDIA on 1800800110 and convey your wish to ‘make an access request’.  

To get access request for children aged 0-6 years, you must approach the NDIS Partner in Community – Early Childhood Early Intervention Partner at https://www.ndis.gov.au/about-us/our-sites/QLD

You will receive an Access request form from NDIA. While you are required to fill in the first parts, Part F should be filled in by your educator, GP or related health professionals.  

The NDIA requires you to submit the form with reports within 28 days.

The NDIA will evaluate your eligibility criteria.

You will be informed of the approval of your access request via letter.

The above table will give you an idea about how your disability impacts on your life, type of support needed and how often you need those services. You can fill out this form to convey your needs to your GP. This information will also be useful while discussing your needs with the NDIA planner.
