Get in touch with the IHDS team to discuss the participant’s goals and support needs for transitioning to Supported Independent Living.

Planning and Coordination
IHDS will collaborate with Occupational Therapists, Support Coordinators, and other professionals to develop a personalised SIL transition plan.

Finding the Right Home and Supports
