Need Help For Applying NDIS? – Access Request Form Available Here!
If you’re a disabled person and not having any disability supports in your area, you will need to contact NDIA & ask for ARF. To help the disabled people during this pandemic, NDIS has made it easier for disabled people to apply for the Scheme.
Disabled people who are eligible for the NDIS can download an Access Request Form (ARF) from our website to apply for the NDIS.
The NDIS Supporting Evidence Form (SEF) is also available online. This form is required for the National Disability Insurance Agency (NDIA) to assess your eligibility needed for the NDIS.
In this form, you will be asked to confirm who you are and need to answer some questions to check whether you meet the NDIS access requirements. You may also be asked for some additional information about your disability. It is essential to provide some information on how your disability affects your ability to perform daily activities. The information you provide will help you get the right care and support that you need.
In this case, as an eligible participant, you can work with your GP, allied health and specialist medical professionals to provide the information that is required.
Disabled people who are eligible for the NDIS can apply the form by following these steps:
- Call 1800 800 110 to get the access request form, or you can even download and complete the ARF from your computer, and email to the NDIA with supporting information.
- You can download the ARF from your computer, and mail it to the NDIA with the supporting information.
- You can email NAT@ndis.gov.au and can get the ARF mailed to you.
- Contact your local NDIA office and ask for an ARF to be mailed.
To download the ARF, you can even contact your Local NDIA or Community Staff to help you apply for NDIS.
