Personalised Hospital Discharge for NDIS participants
I-Help Disability Services’ Clinical Care and Accommodation teams provide comprehensive support for participants transitioning from hospital to supported accommodation. Led by registered nurses, I-Help’s Clinical Care Department ensures specialist care management to all participants.
I-Help Disability Services works closely with hospital discharge teams, social workers, nurses, occupational therapists, and support coordinators to develop a personalised transition plan aligned with the participant’s goals.
Additionally, I-Help Disability Services collaborate with the participant’s family and support network to secure suitable accommodation and tailor support services based on their care needs and preferences.
