Your Transition to Supported Accommodation!

Initial Contact

Get in touch with the IHDS team to discuss the participant’s goals and support needs for transitioning to Supported Independent Living.

Planning and Coordination

IHDS will collaborate with Occupational Therapists, Support Coordinators, and other professionals to develop a personalised SIL transition plan.

Finding the Right Home and Supports

We work with the participant, their family, and support network to identify the most suitable SIL accommodation and services.

Moving In and Ongoing Support

Our team will begin delivering tailored support to help the participant settle in, build life skills, increase independence, and achieve their goals.
